CGnature Medical Clinic Frequently Asked Questions

We accommodate walk-ins most days when our office is open. However, it's best to call us at (708) 295-9399 in advance to confirm availability and make an appointment. You can also schedule an appointment by contacting our team here.

Please download and fill out our Release of Medical Information form to transfer your medical records. You can then present this form to your current healthcare provider or institution. We recommend collecting a personal copy of your records to bring to your appointment. Remember that some places might charge a fee for copying medical records.

We accept a variety of insurance plans. To verify if your insurance is covered, please contact your insurance provider directly or check their website to see if we are listed as a provider. You may also call us at (708) 295-9399 and our friendly receptionist will check if your insurance is covered.

Our services are available to individuals aged 16 and above. We specialize in adolescent and adult care and do not provide pediatric services.

For prescription refills, please have your pharmacy send us a refill request either electronically (e-scribe) or by fax at (866) 920-1134. Requests made after business hours will be addressed based on urgent need and at our healthcare provider's discretion. Please allow us 2 business days for refills.
